Intake Referral Specialist

Conifer Park in Glenville, NY is looking for an Intake Referral Specialist to join our intake department. In this position, you will:

• Complete all necessary intake and insurance information and input data into Meditech
• Request medical records as indicated based on current protocols
• Triage and review records for appropriate placement/admission and make appropriate recommendations of patients
• Document on intake form that patient meets criteria for admission or documents in progress notes referral to alternate level of care
• Communicate with referents, Regional Services, Nursing, Medical, and Administration as needed and documents recommended unit on intake
• Complete pre-cert assessment and communicate with insurance companies/manage care to pre-certify patients for admission when required and documents results of pre-cert to gold communication sheet, communicate pre-cert outcome to patient/family and schedules MD/PA reviews and appeal reviews as needed
• Arrange patient's private payment plans or discuss co-pays and deductibles with family and/or referent and complete payment agreement form
• Consult with senior management if needed to discuss special arrangements
• Schedule patients for admission, document in appointment book, arrange transportation assistance if needed, notify referent of bed date if indicated, review checklist with patient/referent
• Review current pending file, note what needs to take place and make necessary follow-up phone calls
• Perform intake technician duties as needed
• Is responsible for fielding all inquiries, informational calls and admission
• Provide crisis intervention services as needed
• Other duties as assigned

REQUIRED QUALIFICATIONS

• Associates Degree
• QHP required, CASAC, LMHC, LMSW, LCSW, etc.
• Minimum of 1 year experience in chemical dependency, managed care or case management
